About the Program

TheChimo 5 Transition Home is a group care program that aims to support young people learn the skills to live full and complete lives in our community. The young people living at Chimo 5 Transition Home have status with the Alberta government - either Children’s Services or Family Supports for Children with Disabilities (FSCD) or Delegated First Nation Agencies (DFNA). The Chimo 5 program prepares young people to return to living with their families or to transition to adult living situations and adult services (i.e. Persons with Developmental Disabilities, AISH)

Please note: This position requires regular driving within the community. Successful applicants must hold a valid Alberta Driver's Licence, have at least three (3) years of driving experience, maintain a clean three-year Driver's Abstract and Driving Record with no at-fault claims, have access to a reliable vehicle, and provide a currentPolice Information Check andIntervention Record Check as a condition of employment.

What You'll Do:

In this role, you will:

Support youth in building the skills, confidence, and independence needed for adulthood.
Provide individualized case management and ongoing support for your assigned caseload.
Develop, implement, monitor, and evaluate Safety Plans and Service Plans.
Work collaboratively with youth, families, caregivers, service providers, and community partners to ensure coordinated care and support.
Connect youth with community resources that promote long-term success.
Maintain accurate documentation, case notes, and professional reports.
Advocate for youth while promoting strengths-based, person-centred supports.

What You Bring

We're looking for someone who has:

A Diplomaor Bachelor's Degree in Child and Youth Care, Disability Services, Social Work, Psychology, Community Rehabilitation, or a related field.
A minimum of two(2) years of experience supporting children, youth, or young adults.
Experience working with individuals experiencing developmental disabilities, mental health challenges, addictions, trauma, attachment concerns, or barriers to community resources.
Knowledge of community resources available to support youth.
Strong interpersonal, communication, and teamwork skills.
Excellent organizational skills and the ability to write professional reports and documentation.
Confidence using computer systems and electronic documentation.

Requirements

Successful applicants must have:

Eligibility to work in Canada.
A current Police Information Check and Intervention Record Check (issued within the past six months).
A valid Alberta Driver's Licence.
A clean three-year Driver's Abstract and Driving Record with no at-fault claims.
Access to a reliable vehicle.

Physical Requirements

This role requires the ability to:

Lift up to 50 lbs.
Participate in recreational activities and physically engage with youth as required.
