Job Description:Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care. PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills. The PHC Nurse plays a key role in quality improvement in the function of the clinic, primary health supports offered in the community and supports linkages between primary and secondary health care.

Experience

  • Experience in a community based, multidisciplinary setting
  • Previous experience in chronic disease management would be an asset
  • Experience in case management is an asset
  • Minimum one year of recent nursing experience in a clinic-based setting required

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Nursing diploma is acceptable
  • Basic Life Support Certificate (BLS)
