Job Summary:
The primary role of a Youth Support Worker is to build
trusting, supportive professional relationships with youth and young
adults. This role works with a multidisciplinary team to support clients to
help them reach their goals and transition to healthy, confident adults as
per a pre-established plan of care. Working in the Youth & Young
Adult (YAYA) Program, this role will provide youth and young adults with
their basic and immediate needs, ensure their physical and emotional well-being
and safety, support them with relapse prevention and treatment, and help
them to transition back to the community when they may be ready.

Client Services:

  • Welcome

and orient youth/young adults accessing Phoenix services upon entry to the
Program (i.e., introduce and explain services provided, facility
tours, complete program orientation, etc.)

  • Actively

engage with and develop relationships with at-risk youth and young adults
17-24 years of age

  • Coordinate

youth meetings and activities, including recreation and life skills,
individually and in consultation with the primary treatment providers

  • Respond

to and assist with the provision of clients’ immediate needs, including
food, clothing, recreational opportunities, etc.

  • Assist

clients with the completion of instrumental daily activities, including
household upkeep and community participation

  • As

per pre-established care plans, work collaboratively with other staff to
assist youth in attaining short and long-term goals, strategies for
meeting these goals, and an action plan for implementation, based on
inputs provided by the MHSU team.

  • Assist

youth with accessing external community supports and accompany them if
needed

  • Respond

to any medical emergencies or potential behavioral, mental health,
emotional, or substance use concerns using pre-established
trauma-informed practice.

  • Maintain

the cleanliness and safety of the residence, including laundry as
needed

  • Manage

youth case files, including all relevant reporting and documentation

  • Attend

all training and meetings as directed by your manager

  • Complete

room searches and room cleans upon client discharging from program

  • Monitor

site security and respond to safety concerns, including access to/from the
residence, keeping track of clients’ whereabouts, etc.

  • Complete

critical incident reports as per policy

  • Other

duties as required

Qualifications:

  • Grade 12
  • Certificate in Community Social Service or

equivalent

  • Recent, related experience of one year providing

community social services to youth and young adults and working with
youth and young adult populations, substance use services, mental health,
corrections, etc., or equivalent

  • Valid Class 5 driver’s license with clean driver’s

abstract

  • Standard First Aid with CPR-C-AED certificate

preferred

  • Food Safe certificate preferred
  • Satisfactory completion of Nonviolent crisis intervention (NVCI)
  • Food Safe Certification is required
