Job Description:The Social Worker - BSW practices in collaboration with the interdisciplinary team to support clients of the target group(s), significant others and those at risk. The Social Worker - BSW will have a thorough working knowledge of the Saskatchewan HIV Strategy and the Provincial Leadership Case Management Standards. The Social Worker - BSW will incorporate the Four Strategic Pillars of the Saskatchewan HIV Strategy into daily practice.

Education:

Bachelor’s degree in social work

Licenses:

Valid Class 5 driver's license

Membership:

Registered or eligible for registration with Saskatchewan Association of Social Workers (SASW)

Knowledge and Abilities:

Knowledge of the Sask Public Health Act & Communicable Disease Control Regs
Knowledge of social determinants of health affecting vulnerable populations
Ability to work as a member of a multi-disciplinary team
Demonstrates leadership and organizational skills
"Ability to work independently to plan and prioritize workload demands. "
Knowledge in the harm reduction model
Knowledge of client exp. & client support required along the continuum of care

Has demonstrated strong interpersonal and communication skills.

Knowledge of population health approach to health promotion/prevention
Knowledge in principles/practices/concepts of community health/development
